How do I schedule an appointment?

Please complete the new Client intake forms, questionnaires listed on the Client portal. (see link on website). Based on the reason for your visit, you may be asked to complete other forms to help prepare for the visit. We require that paperwork is completed at least 24 hours prior to intake or the session will be cancelled and rescheduled.
